Job Summary Come join the Sage Screening Programs team at the Minnesota Department of Health as a Lead Patient Navigator! The Sage Screening Programs help keep Minnesotans healthy by improving access to breast and cervical screening. The Sage program helps low-income Minnesotans or those who do not have adequate insurance to access breast and cervical cancer screening, diagnostic services and referral to treatment services.
This position provides lead work and guides the Patient Navigation staff, to provide oversight and maintenance of Sage's direct patient recruitment system, assist in the coordination of Sage Programs recruitment activities and screening events.
Key Responsibilities: - Provide leadwork and guidance to Patient Navigation staff to ensure program participants needs are met regarding cancer screening and heart health.
- Assist in the development, maintenance and implementation of systems to support patient navigation activities.
This position is temporary in nature and is anticipated to last through June 27, 2027. Due to the temporary nature, this position can be ended at any time. This position is currently eligible for up to 50% hybrid telework for applicants residing in Minnesota or a bordering state within 50 miles of the work location. Telework of 50% or more may be allowed based on position needs and location. Qualifications Minimum Qualifications The following minimum qualifications must be clearly demonstrated on the applicant's resume and/or application to be considered: One (1) year of experience working in a public health or health-related field in a role of telemarketing, sales, customer service, or appointment scheduling.
Experience must include at least one of the following:
- Compiling, tracking, organizing, and maintaining data.
- Researching, analyzing and/or compiling public health or health-related information.
Minimum Qualifications to be Assessed at Interview: - Verbal communication skills sufficient to communicate clearly with division leadership, staff, and the public.
- Ability to use computers to administer phone scripts and electronic appointment scheduling.
- Knowledge of public health principles, epidemiology, cancer control, and heart health.
- Ability to maintain confidentiality of private information according to laws, rules, policies, and procedures.
- Experience working independently.
Degree Equivalencies: A degree in Public Health, Psychology, Social Work, Nursing, or related field may substitute as follows:
- Doctorate: Two (2) years of experience
- Master's: Eighteen (18) months of experience
- Bachelor's: One (1) year of experience
- Associate's: Six (6) months of experience
Preferred Qualifications - Bachelor's degree in public health, psychology, social work, nursing, or related field.
- Knowledge of state laws pertaining to the collection and protection of medical and legal data, and of federal and state law and statutes regarding data privacy, HIPAA and reporting requirements.
- Ability to establish and maintain working relationships with a variety of partners.
- Ability to speak fluently in Spanish, Hmong, Oromo, Vietnamese, or Somali.
- Experience working in a phone bank or call center.
Additional Requirements This position requires successful completion of a criminal background check and employment reference checks.
This employment does not require a valid driver's license, but it does demand occasional travel, which the candidate will be responsible for meeting.
